LifeWise Health Plan of Oregon was a private, for-profit health insurance company founded in 1986. LifeWise Health Plan of Oregon, often called LifeWise, was part of a group of companies based out of Washington. The company and its affiliates had over 3,600 employees and 2 million members. On April 21, 2016, LifeWise announced that it was exiting the Oregon market. This exit means that all plans sold in Oregon would discontinue on January 1, 2017 and no new Oregon plans would be sold. LifeWise plans sold in different states would continue operating.

Before the Oregon Market exit, LifeWise provided Oregon residents with individual and family health insurance, qualifying HSA, and group health insurance plans.

LifeWise Health Plan of Oregon was accredited by the National Committee of Quality Assurance (NCQA).

Like many health insurance companies, LifeWise Health Plan of Oregon offers PPO and EPO plans.
